Model Notices

The Department of Labor issued model notices on January 13, 2010 to help plans and individual comply with the mandated notifications to certain current and former participants and beneficiaries about the premium reduction. Each model notice is designed for a particular group of qualified beneficiaries and contains information to help satisy the notice provisions.
